Just to show that there are different combinations of the three above:

Action only

Time of Need wrote:

[Action]: You receive 2 movement points and recover 2 [Fatigue].

Exhaust only

Challenge wrote:

Exhaust this card to choose a monster in your line of sight and place your hero token on it. While this card is exhausted, each attack performed by you or the monster, when targeting each other, gains +1 [Damage].

Fatigue only

Nimble wrote:

Each time a monster moves into a space adjacent to you, you may use this card to move 1 space; then the monster may continue its activation. (1 Fatigue)

Action + Exhaust
(I don't see any for this at first glance; correct me if I'm wrong.)

Action + Fatigue

Rage wrote:

[Action]: Perform an attack with a Melee weapon. This attack gains +1 [Damage]. (1 Fatigue)

Exhaust + Fatigue

Prayer of Healing wrote:

Exhaust this card during your turn and choose yourself or an adjacent hero. Roll 1 red power die. The chosen hero recovers [Damage] equal to the [Damage] rolled. (1 Fatigue)

Action + Exhaust + Fatigue

Fury of Undeath wrote:

[Action]: Exhaust this card to activate your Reanimate. Your Reanimate still activates this turn as normal. (1 Fatigue)

None of the above

Accurate wrote:

Each time you perform an attack with a Bow, you may reroll 1 power die (limit once per attack).
